Tell us a pension scheme member has died

Before you start

Gathering some basic information first will help. The form should take about eight minutes to complete.

Afterwards, we’ll ask you to complete a ‘declaration’ with your contact details. We’ll aim to reply within 10 working days.

  • Name of the pension scheme member who has died
  • Their National Insurance (NI) number
  • Their marital status
  • Date of birth
  • Date of death
  • Their usual address
  • Pension scheme name and unique reference number (optional)
  • Your name and contact details
Report through 'Tell Us Once' instead

Tell Us Once is a service that lets you report a death to most government organisations in one go.

You can use Tell Us Once if the person who died was living in England, Scotland or Wales.

A registrar will explain the Tell Us Once service when you register the death. They will either:

  • complete the Tell Us Once service with you
  • give you a unique reference number so you can use the service yourself online or by phone
